The Health & Safety (First-Aid) Regulations (NI) 1982 set out the essential
aspects of first-aid that employers have to address. These regulations apply
to ALL workplaces and state. "an employer shall provide, or ensure that
there is provided, such number of suitable persons as is adequate and
appropriate in the circumstances for rendering first-aid to his employees if
they are injured or become ill at work"

Many thousands of inter hospital transfers and retrievals of both adult and paediatric patients have taken place since the development of a small number of dedicated transfer retrieval teams in the UK in the late 1980’s. This increasing demand for intensive care beds is fuelled by patients and relatives expectations and improved resuscitation and surgical techniques. The usual purpose of inter hospital transfer or retrieval is to allow the patient to be treated more effectively in a geographically separate site. The transfer of patients does not constitute therapy and is not with out risk. It is therefore essential to consider the risk versus the benefits before undertaking a potentially hazardous journey.
 
In 60% of cases the transfer results from lack of a functioning ICU bed in the primary hospital usually as a result of no bed or no nursing staff to look after the patient. The second most common cause (28%) is the requirements for specialist management in a tertiary centre e.g. neuro surgery, burns unit.
 
While transfer can take place from any ward or department in a hospital by far the most frequent is the accident and emergency department or the intensive care unit.
 
Many thousand more intra hospital transfers take place in our hospitals on a weekly basis.
 
Many inter/intra hospital are undertaken by health care professional who are usually poorly equipped and have received little or in most cases no training to overcome the difficulties faced in organising and carrying out the transportation of critically ill or injured patients.  
 
Many transfers are associated with adverse incidents usually the things that we think shouldn’t happen for example oxygen or equipment failure. While both long and short transfer times pose significant risks the latter can more often prove to the most difficult as it is perceived as a short trip e.g. the back of the ambulance to the A&E resuscitation bay or the transfer from ICU to the CT scanner and therefore we are unprepared.  
 
The Star course is aimed at trying to overcome the practical difficulties faced by healthcare professionals. As we use the systematic A, B, C, D, E approach for assessment and treatment of both the paediatric and adult patient STaR employs a unique systematic approach to all patient transfer and retrieval called ACCEPT.
 
The ACCEPT approach stands for Assessment, Control, Communication, Evaluation, Preparing and packaging and Transportation. By following ACCEPT approach the right patient is taken at the right time, by the right people, to the right place by the right form of transport and receives the right care throughout.
